Ascension has Community Health Ministries (CHMs) in five locations across the country, positioned to provide compassionate healthcare and essential social services to underserved populations in urban and rural areas where there is currently no other Ascension presence.

While many programs address specific community needs, CHMs take a broader, localized approach that brings healthcare and social services together to address the unique needs of the specific community.

Our Community Health Ministries

Seton Center | Kansas City, Missouri

Seton Center is a human services agency helping people break cycles of poverty and transform lives. Services include safety-net dental and oral healthcare, a food and basic-needs pantry, emergency rent and utility assistance, a clothing closet, school-based programs, and children’s mental health services.

DePaul Community Health Centers | Gould and Dumas, Arkansas 

DePaul Community Health Centers provides whole-person healthcare and essential social services to individuals and families in Southeast Arkansas. Services include primary care, pediatrics, gynecology, dental care, social services, and community wellness programs.

DePaul Community Health Centers | New Orleans, Louisiana

DePaul Community Health Centers provides affordable healthcare to children, adolescents, adults, and seniors through 11 clinic locations. Services include primary and preventive care, chronic disease management, pediatrics, prenatal care, behavioral health, dental care, optometry, pharmacies, and mobile health outreach.

Centro San Vicente | El Paso, TX

Centro San Vicente provides accessible, comprehensive healthcare to El Paso and surrounding communities, including primary care, dental care, behavioral health support, health education, and 340B pharmacy services. The organization also provides utility and rent assistance, and other support regardless of ability to pay.

Ascension DePaul Services | San Antonio, Texas

Ascension DePaul Services provides community-based programs in San Antonio and Bexar County, including early childhood education, primary medical and dental care, social services, health education, preventive services, and counseling.
