Ascension uses insights from community assessments and social drivers/determinants of health screenings (SDOH) to identify barriers to health and connect people with resources to help address those needs.

Our community health programs help expand access to care, medications, food, transportation, and other essential services.

Neighborhood Resource

Finding local support should not be difficult. Neighborhood Resource is a free online tool that uses a simple ZIP code search to connect people with housing assistance, food resources, transportation, legal services, financial support, and more.

To date, 672,000+ individuals have utilized Ascension’s Neighborhood Resource tool.

Community Health Ministries

Our five Community Health Ministries provide healthcare and social services to communities across the country where access is often limited. Ascension also partners with affiliated Federally Qualified Health Centers (FQHCs) to expand primary, dental, and behavioral healthcare for uninsured and underserved populations.

Through Ascension Rx, patients can receive support accessing affordable medications regardless of insurance status or ability to pay. Our teams are putting medication where it matters most: in the hands of patients in need across our continuum of care.

Rooted in our Mission, Medical Mission at Home brings free healthcare services directly into communities through one-day events offering medical, dental, vision, and pharmacy care – with no insurance, appointments, or payment required.

Maternal Navigation

Ascension’s maternal navigation program helps expectant mothers overcome barriers to prenatal and postpartum care. Maternal navigators connect mothers with transportation, nutrition, childcare, and other essential resources, while providing education and ongoing support throughout pregnancy and after birth.

Since launching in 2019, the program has expanded to six Ascension care sites and supported more than 4,000 mothers facing high-risk births.

Addressing food insecurity

Food access is one of the key needs identified through Ascension’s SDOH screenings.

Nationally, Ascension was one of 14 health systems named to the National Strategy on Hunger, Nutrition, and Health in 2024. We are on track to serve four million individuals by 2030 by expanding screenings, increasing access through community food access points, and implementing solutions that help address food insecurity.
